It will be standard procedure for you to fill in a form online prior to you undergoing any part of the ritual procedure. This will include but is not limited to, your name and date of birth. Some medical declarations to ensure you are a suitable candidate for some aspects of the procedure, your address , phone number and email address are all requested, and a signature to confirm consent to undergo the procedures in the ritual and also an emergency contact please ensure they are happy for me to store their details too.

Purpose of collecting data

To ensure that the client is of sound mind, of a legal age to give consent and to enable periodical communication which may include follow ups to gain feedback, or emails with content to enhance the experience and market further offers and procedures .
